We take pride in the time and specialized care we provide each patient.
Insurance plans often severely limit the time a practitioner can spend with a patient and sometimes the type of care; therefore, Monarch Health, LLC does not participate with in network plans.
We are a membership-based health practice, and are not contracted with any medical insurance companies and are therefore considered "out-of-network."
Payment is due at the time of service. We accept most major credit cards.
We do not bill insurance.
The amount of reimbursement will depend on your specific insurance plan and deductible. We do not guarantee reimbursement.
We encourage you to contact your insurance plan to determine if you will be reimbursed for any of your services.
However, you can utilize your HSA or FSA account.
We do not accept Medicare, Medicaid or Tricare. You may NOT file a claim with any of these government health plans as Dr. Jaclyn Piasta, DNP has opted-out of these plans.

Concierge care means your clinician actually knows you, and because knowing you is my priority, I have a limited number of patients.
As a patient, you can reach me directly anytime. Myself or one of our nurses will answer you personally.
I treat what's bothering you now, coordinate with others on your healthcare team, and stay invested in where your health is headed. Visits are in depth and extended in time.
My goal is for you to feel truly taken care of.

Starting January 2026, new legislation will allow for expanded use of some FSA/HSA funds for memberships. This change will further enhance the flexibility and convenience of managing your healthcare expenses with Monarch Health. We encourage you to contact your insurance provider to learn more about your options.

We currently offer in-person appointments in our Marietta, GA office and telehealth appointment for individuals living in Arkansas, Florida, Georgia, and Texas.

Yes, we have in office lab work capabilities for our members only. We are contracted through LabCorp and will bill via insurance.
We also have an account with Quest Diagnostics and can order for lab draws at their patient service centers.
Their phlebotomy centers are located nationwide and will be able to bill your labs through your medical insurance provider.

Yes, absolutely. I don't bill any insurance, and Medicare is no exception, so your membership works exactly like everyone else's: you pay for your care directly.
Federal rules do require that you and I sign a short agreement confirming you understand Medicare won't be billed and won't reimburse you for my care. It takes two minutes and happens alongside your membership agreement.
Finally, please keep your Medicare coverage overall. Regardless of your membership status with my practice, Medicare works for many other needs like hospital care, labs, imaging, prescriptions, and your other doctors.

Yes, absolutely. I don't bill any insurance, and Kaiser is no exception, so your membership works exactly like everyone else's: you pay for your care directly.
However, please note that Kaiser insurance does not cover labs ordered by an outside clinician.
If you wish for Monarch Health to order your labs, we highly discounted cash pay lab work through Quest.

To keep you safe and fully supported, membership does not replace a few things: emergency or urgent care (always call 911 or go to the nearest emergency department for those), obstetric or prenatal care, or hospital care.

It depends.
For many patients, we can serve as their primary care provider. For those with complicated medical histories, we ask may ask that you maintain a relationship with another primary care clinician with more expanded capabilities.

No, labs are not included in your membership fees.
In most cases, labs are billed through your insurance by one of the lab providers (Labcorp/Quest).

It depends. We offer limited gynecologic care at our office including but not limited to contraception, minor procedures such as biopsies and IUD insertions, cultures for infections.
We are also able to coordinate mammography and other screening orders as well as certain gynecologic imaging if needed.
If you require more advanced gynecologic care such as colposcopy and/or gynecologic surgery, we will need to refer you to an outside practice.

Yes! That is one of the main focuses of our practice. We prescribe a myriad of hormone therapies including estrogen, progesterone, testosterone and thyroid. We do not offer testosterone and estrogen pellets.

Yes, when appropriate, we prescribe all available pharmaceutical options for obesity and weight management—including GLP-1 medications such as Wegovy and Zepbound.

Our practice does NOT support emergency or urgent care needs. Always call 911 or go to the nearest emergency department if you have an urgent medical concern or a experiencing a health emergency.
Please note our electronic medical record platform and phone systems are to set up for immediate response times. Text message inquires are subject to the same response time as all other forms of communication.

Health and wellness is a commitment. We are interested in working with individuals who align with this principle. With that being said, we have no long-term commitment.
We ask for a 6 month commitment. Most of our patients end up staying longer - because we do things differently here!

Our membership is not so that you need to see us frequently. It is so that you have peace of mind, ease of access to your provider, and a commitment on our behalf to providing evidence-based and compassionate care to you. Life is unpredictable, and you can’t predict what health concerns may come up, no matter how healthy you are.

This is simple. You can text us on the secure patient text line or send a message via the patient portal.
For non urgent notes, you can expect a reply within about two business days.
Because this is a solo practice, I do not offer 24/7 or after hours coverage, so anything urgent or an emergency should go to 911 or your nearest emergency department.
