Please follow the steps below if you are interested in becoming a new patient.
Step 1: Initial Consultation
At Monarch Health, your care begins with a personalized, two-part experience designed to uncover the root causes of your symptoms and create a clear path forward.
Part A: Pre-Visit with Our Nurse, Megan
Your care begins with a dedicated pre-visit led by our RN, Megan.
During this session, we will:
Review your medical history and current medications
Coordinate and order lab work, if clinically appropriate
Offer an optional in-office InBody® scan and results review to assess your body composition
Part B: Comprehensive Consultation with Dr. Jackie Piasta
Your 60-minute consultation with Dr. Piasta is where the transformation begins. Together, you’ll:
Review your detailed health history and intake
Explore your symptoms, lifestyle, and goals in depth
Discuss potential root causes and evidence-based treatment options
Review lab results and ensure complete understanding
Collaborate on a personalized care plan and begin treatment, when appropriate
This visit lays the foundation for your individualized midlife and menopause care plan.
Investment: $500
Please note: New patient appointments are currently booking 3–4 months in advance.
To reserve your new patient experience, a $150 scheduling fee is collected at booking. This fee secures your appointment and covers the nurse pre-visit consultation (Part A). Please note, the fee is non-refundable within 48 hours of your scheduled appointment with our nurse.
Our office will reach out to schedule your nurse pre-visit after appointment booking.
Step 2: Enroll in Membership
After your initial consultation, you’ll have the opportunity to enroll in our concierge membership.
Membership includes:
Direct access to the Monarch Health team via our portal and member text line
Personalized medical guidance, testing and follow-ups
Symptom tracking and treatment plan updates
Exclusive access to our Member Center and curated education and virtual events
Timely, coordinated care designed for midlife and beyond
Other miscellaneous health needs, such as urgent care, virtual visits, and prescriptions.
Membership fees start at: $175/month
Initial Consultations are needed to begin your membership with Monarch Health and are a one-time, 60-minute visit.
To move forward with care following your initial consultation, including prescriptions or follow-up care you must be an enrolled member.
Monthly membership fees begin one month after your initial consultation.
A 4-month minimum commitment ensures continuity and effectiveness of care.
In-person consultations are located at our office in East Cobb, Marietta, Georgia.
Patients who live in Georgia, Florida, Texas, and Arkansas can be seen virtually via Telehealth.
