What to expect at Monarch

  • Let’s Get Started

    Your visit includes a nurse-led pre-visit to coordinate lab work, followed by a comprehensive 60-minute consultation with Dr. Piasta. Together, you will review your health history, concerns, and goals, and develop a personalized plan of care tailored to your needs.

    Patients who wish to continue care with Jackie Piasta, NP, and Monarch Health beyond the initial consultation are required to enroll in our membership. Membership begins the month following the initial visit.

    New patient appointments are currently scheduling 2–3 months in advance.

    Initial Consultation Fee: $500

  • Follow-up visits are typically 30–40 minutes and may be conducted either in person or virtually.

    Ongoing follow-up care is reserved for our membership patients.

    For individuals who do not plan to continue with the practice, a one-time follow-up visit after the initial consultation is available for $450.

  • Medically supervised weight loss plans available with our concierge membership tiers.

    *Medication costs not included.

  • We are offering educational consultations for individuals located out of Arkansas, Georgia, Texas and Florida.

    This visit is ideal for those who would like to learn more about their health or a particular health condition.

    Educational consults typically last between 45 minutes and up to 1 hour in length.

    *These visits do not involve the prescribing of medications.

  • Included with membership

    Courtesy prescription refills, referral to pelvic floor physical therapist (GA patients), vacation medication needs, or minor medical concerns such as cough, cold, flu symptoms.

  • Our medically formulated IV infusions provide rapid nutrient absorption to boost energy, strengthen immunity, and promote overall wellness—all delivered safely in a relaxing, restorative environment.

  • We offer a yearly well woman visit in our inviting office room for our membership patients. This includes pap smear collection (if indicated) and ordering of routine lab work, both of which are billed to your medical insurance.

  • We offer limited gynecologic care at our office including but not limited to contraception, minor procedures such as biopsies and IUD insertions, cultures for infections.
